Legal Name Change Documents
If you have legally changed your name, then you will be required to provide a certified copy of:
- a legal change of name document;
- court order;
- adoption order;
- marriage certificate issued by civil authorities with your new name; or
- a divorce decree with your new name.
Please note that Citizenship and Immigration Canada will not accept marriage and legal change of name certificates issued by the Government of Quebec prior to January 1, 1994. If you have such a document, you must obtain a new document issued by the civil authorities in the province of Quebec dated on or after January 1, 1994.
Documents Requiring Translation into English or French
If any of the documents that you are required to submit with your application are not in English or French then you are required to provide the following:

a certified copy of the document to be translated;

the English or French translation of the document; and
- a signed translator's declaration that states: (i) the translator's name; (ii) the original language of the translated document; and (iii) a statement by the translator that the translation is accurate.
